Elderly Dog Waiver

At DOGGIE DAYS, we are happy to care for senior dogs.  However, due to the increased risks associated with advanced age, we require this waiver to be signed for any dog 8 years of age or older (or otherwise considered a senior).

You will be asked to sign the agreement upon your next check-in for Daycare, Boarding, and Grooming.

I understand that senior dogs may have underlying or age-related medical conditions that may not be immediately apparent.  

I acknowledge that boarding, environmental changes, or interaction with other dogs may increase these risks.

I confirm that I have disclosed all known medical conditions, medications, mobility limitations, behavioral issues, and recent veterinary concerns related to my dog.

I understand that DOGGIE DAYS will make reasonable efforts to monitor my dog and notify me if concerns arise.  However, senior dogs can decline quickly, and not all medical events are predictable or preventable.

If my dog becomes I'll or injured and I cannot be reached, I authorize DOGGIE DAYS to seek veterinary care at their discretion.

I understand that all veterinary costs are my(owner's) responsibility.

In the event of a life-threatening emergency where immediate decisions must be made and the owner cannot be reached, I authorize the attending veterinarian to provide necessary treatment deemed appropriate for the welfare of my dog.

I understand that older dogs may be more prone to falls, joint strain, fatigue, difficulty getting up or navigating surfaces, and other issues.  While DOGGIE DAYS takes reasonable precautions, I acknowledge these risks cannot be completely eliminated. 

I acknowledge that due to my dog's advanced age, there is an increased possibility of illness, injury, or death during boarding.

By signing the form at the kennel, I agree not to hold DOGGIE DAYS, it's owners, employees, or agents liable for age-related illness, injury, or death that may occur during my dog's stay, provided reasonable care is exercised.
